Upward Bound Forms and Applications
Thank you for your interest in the Upward Bound program at Andrew College. This page will provide you with all of the forms necessary for a student to enter the program. If you have any questions about the application process, don't hesitate to contact us at 229.732.5903.
You will need Acrobat Reader to access the forms below. Reader is a free program that can be downloaded from Adobe's website in two quick and easy steps. Click here to get Acrobat Reader.
Your application must include ALL of the following items before it can be accepted for consideration.
- Student Information and Application Form
- Parent/Guardian Information Form
- Family Income and Support Information Form (accompanied by a copy of the parent(s) most recent tax return form - 1040EZ, 1040A, 1040)
- Family Size and Taxable Income Form
- Teacher Recommendation Form (completed by a teacher in the core curriculum - ex., Math, Science, English, History, or Foreign Language)
- Student Essay Form
- A copy of the student's most recent report card (if report cards have not been issued then a mid-term progress report will suffice until a grade report can be supplied)
